What does this number tell me 5J8483, it's on a Cat D2 with a rear mounted hydraulic pump for the front blade. Is this a factory blade set up? Thank you. I've been tasked to sell this gas fifteen also.

look at this serial numbers list, blade kinda looks like l-p trail blazer

While it looks like a LaPlant, I don't think it is. The reason is the frame on the under carriage is welded plates. An LP has all formed steel. I am sure its aftermarket but whose? Not exactly sure. A Dakota maybe?

After looking at a YouTube video the LaPlant-Choate blade sits further back then this one?

Also LP's tilt as well as angle.
You are indeed correct in that looking at the first photo, the blade is way out in front.
I've seen HOLT blades with that metal block sticking out of the top of the blade.
