First suspect would be a precombustion chamber.
Can you start it to check which cylinder is not firing.?

I've been working on a d4 d-4400 engine that sat under a tree for a while and had the frost plugs rust through allowing a large quantity of water to get into the oil. This is easily checked by removing the rocker cover. You said oil in the exhaust sounds more like precombustion chamber seals or a bad head gasket. Hopefully not the worst possibility of a cracked head/block. What model d4 are you working on?

Figure out which cylinder is missing by loosening each injector line in turn. The one that affects the idle the least or not at all is your best bet. If you can't or don't want to run it, you can remove the injectors and put compressed air into each cylinder with the piston at TDC of compression stroke. Watch for bubbles in the radiator. Once you have it narrowed down I would remove the precombustion chamber and inspect. If it is rusted out, the other 3 probably are too. New are available. You might just need to replace the seal washers underneath.
D2-5J's, D6-9U's, D318 and D333 power units, 12E-99E grader, 922B & 944A wheel loaders, D330C generator set, DW20 water tanker and a bunch of Jersey cows to take care of in my spare time😄

No wrench needed; find a bolt head that fits the plug and weld a nut onto your bolt. Viola, instant oil plug tool. Doubles as a final drive plug tool.

I really like all the resources on here so I just got a three year. I think its a very small price for what you can get out of this site."
